Joint Request for Arbitration
Arbitration is a process whereby a Neutral renders a decision after hearing an abbreviated version of the evidence and depending on the agreement of the parties, may or may not be binding.
Parties may file a joint request using the forms below and submitting the non-refundable $500 ($600 for non-CAI members) filing fee.

Joint Request/Agreement for Arbitration Form
Step 1: Initiator and Respondent must complete and return with fee the joint request/agreement for Arbitration Form
Initiator's Statement of Dispute Respondent's Statement of Dispute
Step 2: Initiator and Respondent must complete and sign the statement of dispute and identify all parties involved in the dispute.
Initiator's Notice of Legal Representation Respondent's Notice of Legal Representation
Step 3: Either party may be represented by counsel but must give advance notice to all parties if they elect to be represented.
